Executed elements must be given in the order in which they occur in the program. Please fill out one PPC for each category that you are going to participate in. Please make sure you select the correct category for each PPC, however, no PPC is required for Artistic programs and the Pattern Dances.
Pairs: You only need to fill out one form per pair.
